About you: In your new role you will…… Be self‑directed and show initiative under the guidance of the Team Leader and Aboriginal and Torres Strait Islander Health Worker Cluster Coordinator as Professional Lead. Develop, deliver and evaluate primary health care programs for Aboriginal and Torres Strait Islander communities, ensuring programs are responsive and consistent with community needs, values and beliefs. Actively participate in weekly multidisciplinary case conference and triage under direction of Registered Nurse / CNC.
