About you: In your new role you will: Empowering consumers to make informed health care decisions through consultation, education and information sharing. Act as a cultural liaison to advocate, liaise, network and refer to appropriate Non-Government Organisations, Aboriginal and Torres Strait Islander Liaison Officers and the Multidisciplinary Team in collaboration with and under direction of Registered Nurse. Provide effective support and advocacy for identified Aboriginal and Torres Strait Islander Rapid Response Falls Service clients by conducting timely home visits, case management/conferencing, follow-up and referrals to relevant health services/programs.
