Key Responsibilities: Client Support: Provide tailored support to individuals with chronic health conditions, including heart failure, respiratory conditions, and neurological disorders.Address the needs of vulnerable individuals and priority population groups, ensuring equitable access to care and resources.Collaborative Practice: Actively participate in weekly multidisciplinary team (MDT) case reviews, contributing to coordinated care planning and decision-making.Work closely with a diverse team of healthcare professionals to provide holistic care.Quality Improvement: Lead and participate in quality development projects to enhance service delivery and outcomes for patients.Identify opportunities for service innovation and implement evidence-based improvements.Professional Development: Engage in ongoing professional development opportunities to enhance skills and knowledge.Access resources and training to support personal and professional growth.Opportunities: Be part of a dynamic team focused on delivering high-quality, person-centered care.Contribute to initiatives that make a meaningful difference in the lives of patients with complex health needs.Engage in leadership and quality improvement projects to shape the future of chronic care services.This role offers a chance to make a significant impact by supporting individuals to navigate their healthcare journey and improve their quality of life.

Selection Criteria: Personal integrity and demonstrated commitment to the Philosophy, Mission and Values of Mary Aikenhead Ministries and St Vincent's Health Australia.Bachelor of Social Work or equivalent tertiary qualification creating eligibility for membership of the Australian Association of Social Workers (AASW) with demonstrated experience in clinical health social work settings with high level skills in aged care social work service.Demonstrated competencies in relevant social work practice frameworks including human rights, social justice, crisis intervention, adjustment to illness and disability, short term solution focused intervention, grief and loss, complex assessment, advocacy, care and discharge planning.High level understanding and experience of the complex social issues for patients, families and carers in regard to aged care services, rehabilitation, disability and residential care.Demonstrated commitment to and experience in inclusive collaboration and multidisciplinary teamwork in high demand environment including time management, negotiation, written, oral and computer skills.Demonstrated competencies and experience in the provision of clinical supervision of staff/students.
Ability to undertake and lead clinical planning, research, quality and education projects within social work services.Sound knowledge of legislative requirements and the social work role in regard to domestic violence, elder abuse, child protection, guardianship and victims of crime.Current unrestricted NSW Class C Driver's LicenseShortlisting for this position will commence immediately.
